Overview

Become an integral part of our dedicated team at Alto Health Care Staffing. As a vital contributor, you will deliver therapeutic services aimed at assisting clients in regaining or enhancing their ability to perform daily tasks independently. Your role will profoundly influence the well-being of those you assist.

Key Responsibilities
  • Conduct comprehensive initial and ongoing assessments of clients, including admissions, service recertifications, and discharges.
  • Supervise the care provided by Physical Therapy Assistants (PTAs).
  • Administer physical therapy interventions in accordance with physician directives and regulatory standards.
  • Promptly inform physicians of any changes in client conditions and secure necessary orders for adjustments to care plans.
  • Assess and modify equipment and environments to align with client requirements.
  • Educate clients and caregivers to foster both physical and emotional wellness.
  • Ensure timely and precise documentation of visits and client interactions by the following morning, including:
    • Initial Assessments
    • Recertifications
    • Discharge Summaries
    • Missed Appointments
    • Communication Records
  • Respond efficiently to inquiries and requests from the administrative team.
  • Participate in mandatory meetings and facilitate service coordination through effective communication with the healthcare team.
  • Maintain a caseload that meets agency productivity benchmarks in line with best practices.
  • Address referrals for physical therapy promptly and document care in accordance with agency protocols.
  • Uphold client dignity, privacy, and confidentiality of personal information.
  • Ensure that client rights are safeguarded and that all clients and caregivers are informed of their rights and responsibilities.
  • Initiate emergency protocols as necessary.
  • Guarantee that clinical and administrative documentation complies with agency, professional, and regulatory standards for accuracy and timeliness.
  • Perform additional duties as assigned by management within the scope of practice.
Communication and Authority
  • Deliver clear, concise, and relevant information in a non-judgmental manner.
  • Maintain open communication channels with all team members.
  • Employ effective problem-solving and conflict resolution techniques.
  • Encourage supportive team dynamics.
Professional Accountability
  • Exhibit knowledge of relevant Physical Therapy standards in accordance with Medicare Conditions of Participation.
  • Adhere to and enforce all agency policies and procedures.
  • Demonstrate effective prioritization and time management abilities.
  • Carry out responsibilities independently, without supervision.
  • Promote the agency and its services to clients.
  • Display a caring attitude when interacting with clients, families, and colleagues.
  • Comply with ethical standards and demonstrate proficiency in assessments and treatment methodologies.
  • Ensure timely, accurate, and appropriate documentation of care and communications related to patient care.

Qualifications
  • Graduate from an Accredited Physical Therapy Program.
  • Current licensure and good standing in the State of Colorado.
  • Prior experience in home care settings.
  • Valid driver's license and auto insurance.
Dress Code
  • Attire should be clean and free from damage.
  • Closed-toe footwear is mandatory in the field.
  • Clothing with potentially offensive imagery or language is prohibited.
  • Scrubs and business casual attire are acceptable.
Technology Utilized
  • Proficient in Microsoft applications including Outlook, Teams, and Excel.
  • Experience with Sansio EMR.
Equipment Provided
  • iPad for documentation and communication.
Reporting Structure
  • Reports to the Clinical Director and Senior Director of Quality.
Work Schedule

Standard hours are Monday through Friday, 8 am to 5 pm.

Licensure Requirements

Must hold an active license.

Minimum of 1 year of experience required.
