Objective


Join a dedicated team in delivering therapeutic services aimed at assisting clients in regaining or enhancing their ability to perform daily activities independently, within the professional scope of practice.

Key Responsibilities

  • Conduct initial and ongoing assessments of clients, including evaluations for care initiation, service recertification, and discharge planning.
  • Supervise the care provided by Physical Therapy Assistants.
  • Deliver physical therapy services in accordance with physician directives and regulatory standards.
  • Promptly inform physicians of any changes in client conditions and secure necessary adjustments to care plans.
  • Identify and implement necessary modifications to equipment and environments to cater to client needs.
  • Educate clients and caregivers to foster both physical and emotional well-being.
  • Ensure accurate and timely documentation of visits and client communications, to be submitted by the following morning, including:
  • Initiation of Care
  • Service Recertifications
  • Discharge Planning
  • Missed Appointments
  • Communication Records
  • Respond to inquiries and requests from the administrative team in a timely manner.
  • Participate in required meetings and facilitate effective communication with other healthcare team members.
  • Maintain a caseload that meets agency productivity standards, adhering to best practices.
  • Respond to referrals for physical therapy promptly and document care in accordance with agency protocols.
  • Uphold client dignity, privacy, and confidentiality of personal information.
  • Ensure client rights are respected and that all clients and caregivers are informed of their rights and responsibilities.
  • Initiate emergency protocols as necessary.
  • Ensure clinical and administrative documentation complies with agency, professional, and regulatory standards for timeliness and content.
  • Perform additional duties as assigned within the scope of practice.

Qualifications:

  • Graduate of an Accredited Physical Therapy Program.
  • Current licensure in good standing.
  • Prior experience in home healthcare settings.
  • Valid driver's license and auto insurance.
