Pharmaceutical Care Specialist, Flexible Schedule

Pharmaceutical care specialists play a crucial role in ensuring the safe and effective delivery of medications and related healthcare products necessary for optimal patient outcomes, adhering to established professional standards and practices. These specialists are also tasked with monitoring medication therapies, providing patient counseling, and offering guidance to healthcare providers and nursing teams regarding pharmacotherapy.
Key Responsibilities
The responsibilities outlined below reflect the typical tasks performed on a daily basis. Additional responsibilities may be assigned as required to support the organization's vision, mission, and core values.
- Evaluate and confirm prescriptions and medication orders for suitability, focusing on safety and effectiveness.
- Conduct proactive drug utilization assessments, ensuring proper dosages, usage instructions, potential drug interactions, and any other relevant treatment considerations, intervening as necessary.
- Oversee pharmacotherapeutic services in alignment with institutional guidelines and protocols.
- Monitor medication regimens for safety and effectiveness, making necessary interventions, including applying pharmacokinetic principles when appropriate.
- Educate physicians, advanced practice providers, nursing staff, pharmacy personnel, and other relevant parties as required.
- Collaborate with procurement staff to assist in inventory management and adherence to inventory protocols.
- Maintain accurate records for all medications managed within the pharmacy.
- Perform verification of medication products, including the preparation and review of hospital and emergency response kits, intravenous admixtures, unit dose medications, and other pharmaceutical products.
- Adhere to established departmental policies, procedures, objectives, quality assurance programs, and safety, environmental, and infection control protocols.
- Support the education of pharmacy residents and students, including facilitating learning experiences and assignments as necessary.
- Consult with patients and their families or caregivers regarding all aspects of medication usage, including potential risks, side effects, and drug composition as needed.
- Exhibit proficiency in pharmacy automation, medication distribution, and relevant technologies applicable to the work environment.
- Contribute to departmental cost-saving initiatives and/or revenue enhancement efforts.
- Participate in or lead departmental quality improvement initiatives (including at least one medication use evaluation, presentation, or quality improvement project annually with approval from the inpatient pharmacy manager).
- Attend mandatory hospital-wide orientations, meetings, and training sessions.
- Demonstrate a willingness to adapt work schedules flexibly to ensure comprehensive patient care.

Education and Experience
Minimum Required Education: Bachelor of Science in Pharmacy
Preferred Education: Doctor of Pharmacy from an ACPE-accredited institution, completion of ASHP-accredited PGY1 Pharmacy Residency
Minimum Required Experience: None
Preferred Experience: 1 year of experience in inpatient pharmacy practice
Licenses and Certifications
Minimum Required: Must hold a valid Pharmacist license in the Commonwealth of Virginia. Basic Life Support and Advanced Cardiovascular Life Support certifications are mandatory.
Preferred: Relevant certifications or board certifications from the Board of Pharmacy Specialties are highly encouraged.
