Overview

Pharmaceutical Care Specialists play a crucial role in the healthcare system by ensuring the safe and effective use of medications to promote optimal patient outcomes. They are tasked with monitoring drug therapies, providing patient counseling, and collaborating with healthcare professionals to enhance medication management.

Key Responsibilities

The following responsibilities outline the essential functions of this position. Additional tasks may be assigned to align with the organization's mission and values.

  • Evaluate and confirm prescriptions and medication orders for appropriateness, ensuring safety and effectiveness.
  • Conduct proactive drug utilization assessments, considering proper dosages, administration instructions, potential drug interactions, and other relevant factors, intervening as necessary.
  • Oversee pharmacotherapeutic services in accordance with established institutional standards and protocols.
  • Monitor medication regimens for safety and effectiveness, making necessary adjustments based on pharmacokinetic principles when applicable.
  • Provide educational support to physicians, advanced practice providers, nursing staff, pharmacy personnel, and other relevant parties.
  • Assist with inventory management in collaboration with procurement staff, ensuring compliance with inventory protocols.
  • Maintain accurate records for all medications managed within the pharmacy.
  • Perform verification and preparation of all medication products, including hospital and emergency kits, IV admixtures, and unit dose medications.
  • Adhere to established departmental policies, procedures, objectives, and safety protocols.
  • Support the learning and development of pharmacy residents and students through mentorship and educational experiences.
  • Consult with patients and their families regarding medication use, potential risks, side effects, and drug composition.
  • Exhibit proficiency in pharmacy automation, medication distribution, and relevant technologies.
  • Contribute to departmental cost-saving initiatives and revenue enhancement efforts.
  • Participate in or lead quality improvement projects within the department, including at least one medication use evaluation annually with the approval of the inpatient pharmacy manager.
  • Attend mandatory orientations, meetings, and training sessions as required.
  • Demonstrate flexibility in work scheduling to ensure continuous patient care.

Qualifications

To excel in this role, candidates must meet the following qualifications:

Education and Experience

Minimum Required Education: Bachelor of Science in Pharmacy

Preferred Education: Doctor of Pharmacy from an accredited pharmacy school, completion of an ASHP-accredited PGY1 Pharmacy Residency

Minimum Required Experience: None

Preferred Experience: One year of experience in an inpatient pharmacy setting

Licenses and Certifications

Minimum Required: Must hold a valid Pharmacist license in the Commonwealth of Virginia. Basic Life Support and Advanced Cardiovascular Life Support certifications are mandatory.

Preferred: Additional certifications or board certifications from the Board of Pharmacy Specialties are highly regarded.
