The Senior Care Physician (PCP) serves as a pivotal leader within our collaborative care framework. Our healthcare team includes Physicians, Advanced Practice Providers, Pharmacists, Care Coaches, Medical Assistants, Behavioral Health Specialists, Quality Coders, Referral Coordinators, and more. Our model is designed to enhance care quality and achieve better health outcomes for seniors by providing a personalized experience, multidisciplinary services, coordinated care backed by analytics and tools, and strong community ties. This extensive support allows our PCPs to manage fewer patients while dedicating more time to each individual.

Key Responsibilities:
  • Assess and treat patients in alignment with established care standards.
  • Monitor the quality of care provided to patients through data analysis and chart evaluations.
  • Facilitate the coordination of patient services, including specialty referrals, hospital and skilled nursing facility coordination, durable medical equipment, and home health services.
  • Actively engage as a key medical resource within the care team during daily meetings.
  • Support the Regional Medical Director and Center Administrator in fostering a collaborative practice environment through professionalism and approachability.
  • Ensure timely completion of medical documentation, collaborating with quality coders to enhance coding accuracy.
  • Adhere to policies and protocols established by Clinical Leadership.
  • Meet regularly with the Regional Medical Director to discuss care quality, outcome reviews, and policy matters.
  • Engage in potential growth initiatives for new or existing services within the Center.
  • Participate in the local primary care on-call program as necessary.
  • Maintain compliance with licensing, certification, and accreditation requirements.
  • Devote 100% of your time to direct patient care, including patient interactions and necessary administrative tasks (charting, meetings, etc.) related to patient care.
Utilize Your Expertise to Make a Difference

Required Qualifications:
  • Current and unrestricted medical license or willingness to obtain a medical license in the state of practice; eligible and willing to secure licenses in other states as needed.
  • Graduate of an accredited MD or DO program.
  • Exceptional verbal and written communication abilities.
  • Strong interpersonal skills and the ability to communicate effectively with colleagues and patients.
  • Fully committed to the concept of integrated team-based care.
  • Willingness to learn and adapt to a value-based care environment.
  • Outstanding patient/customer service skills.
  • Basic computer proficiency, including email and electronic medical records.
  • This role involves patient interaction and is part of our Tuberculosis (TB) screening program; selected candidates will be required to undergo TB screening.
Preferred Qualifications:
  • Board Certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine, or Geriatric Medicine is preferred.
  • Active and unrestricted DEA license.
  • Medicare Provider Number.
  • Medicaid Provider Number.
  • Two to five years of relevant experience is preferred.
  • Experience managing a Medicare Advantage patient panel with knowledge of best practices in a coordinated care environment.
  • Familiarity with Medicare guidelines and coverage.
  • Bilingual skills are a plus.
  • Understanding of HEDIS quality indicators.
