Key Responsibilities- Provide support and supervision to adults with developmental disabilities in their home and community, including assistance with personal care, household duties, and skill building.
- Develop and implement individualized plans of care to meet the unique needs of each resident.
- Document and communicate resident progress and needs to management and other team members.
- Monitor and communicate the ongoing health and safety needs of residents.
- Assist residents with daily living tasks, including bathing, toileting, and meal preparation.
- Support residents with social and leisure activities, including transportation and outings.
- Administer and document the administration of resident medications.
- Provide awake overnight supervision to residents, responding to resident needs as necessary.
- Collaborate with resident, family, and other service providers to ensure seamless care and support.
- High School Diploma or GED.
- Some experience working with adults with developmental disabilities and/or mental illness preferred.
- Knowledge of person-centered practices and techniques.
- Ability to communicate effectively, orally and in writing.
- Ability to establish and maintain effective working relationships with residents, their families, other staff, and significant others.
- Good physical condition, including the ability to physically assist participants who may be of limited mobility.
