Position Overview

Key Responsibilities of the Hospice LPN:

  • Report directly to the Administrator or Executive Director.
  • Deliver direct patient care services to hospice patients while providing support to their families with a manageable case load.
  • Assess patient and family needs, contributing to the development and implementation of personalized care plans.
  • Communicate any changes in the patient's condition to the Registered Nurse, interdisciplinary team, and referring physician, ensuring thorough documentation.
  • Participate in staff meetings to enhance team collaboration.

Candidate Profile

Qualifications for the Hospice LPN:

  • A compassionate heart, demonstrating kindness and empathy towards patients and their families.
  • Comfortable providing care in various home settings.
  • Ability to maintain composure and flexibility while supporting others during challenging times.
  • Valid Licensed Practical Nurse (LPN) license in the state of employment.
  • A minimum of 1 year of patient care experience in hospice, home health, ICU, geriatric, or oncology settings is strongly preferred.
  • Additional preferred experience may include roles such as hospital Staff LPN, Staff LVN, Med Surg, PACU, Float pool, ER, critical care, acute care, cardiac, geriatric, admissions, wound care, telemetry, nursing home, or travel nursing.
  • Current driver's license and automobile insurance.
  • Up-to-date CPR certification.
