Job Description

Greater Baden Medical Services, Inc. is seeking a highly skilled Registered Nurse Program Coordinator to join our team. As a clinical and managerial resource, you will play a crucial role in delivering medical services in a safe and efficient manner, ensuring that patients receive the best possible care.

The RN Program Coordinator will lead the FOCUS program and work as the nursing lead in the GBMS HIV program. This role requires strong leadership and communication skills, as well as the ability to work effectively in a team environment.

Key Responsibilities

  1. Manage and improve the FOCUS program in collaboration with the CMO, Chief Administrative Officer, quality team, IT data analyst, and providers.
  2. Support Primary Care Provider teams to ensure HCV/HIV tests are ordered routinely on appropriate patients, providing educational updates.
  3. Care manage clients known to be HIV-positive and HCV-positive clients in coordination with case managers, Infectious Disease Providers and other staff.
  4. Participate in meetings with Gilead program officer.
  5. Monitor data collection reports for accuracy and completeness, providing quarterly updates to quality teams.
  6. Responsible for narrative sections of reports and supplies information and data for program reports and relevant new grant funding/projects.
  7. Identify needed policies and procedures to improve processes and program initiatives.
  8. Communicate and promote programs at provider meetings and other meetings on a periodic basis.
  9. Serve in collaboration with providers and health care teams to maximize treatment adherence, and to resolve issues compromising compliance and patient well-being.
  10. Participate in monthly team meeting with to review quantity/quality goals for all staff on these grants.
  11. Become proficient in and utilize the Electronic Health Records database to track patient treatments, laboratory results, patient self-management goals, schedule appointments, provide referrals, and perform other functions needed for patient care.
  12. Become proficient or gains proficiency in telehealth platforms supported by the organization.
  13. Work to link 100% of newly diagnosed HIV patients to RW program (about 15 per year).
  14. Work to link 100% of new diagnosed HCV patients to GBMS infectious diseases service (about 30 per year).
  15. Participate in partnership Coalition.

Essential Functions—GBMS HIV Program

  1. Serve as nurse and care manager for patients living with HIV.
  2. Provide provider support for the Infectious Disease provider team for patients who are not eligible for RW services.
  3. Help improve, develop and write clinical policies and procedures.
  4. Participate with audit preparation.
  5. Assist in identifying training needs and coordinating training for staff.
  6. Provide patient education in nurse-only visits.
  7. Become proficient in remote patient monitoring initiatives.
  8. Help manage 300+ RW infectious disease patients.
  9. Connect 300 RW ID patients under care to MCM and NMCMs in RW program.
  10. Ensure 50% of patients living with HIV are referred to dental in coordination with team
  11. Ensure 25% of patients living with HIV are referred to Behavioral Health in coordination with team
  12. Attend weekly Ryan White team meetings.

Other Essential Functions

  1. Provide direct nursing care to patients, including performing assessments, nursing procedures, and assistance with procedures.
  2. Provide health teaching and counseling to patients and families. This may include communication of test results, medication changes, etc.
  3. Assess patient requests for same day services, and work with Providers and Center Manager to insure that patient needs are addressed appropriately.
  4. Demonstrate proficiency in the use of the EMR, serves as a clinical resource.

Minimum Qualifications

  1. Current Registered Nursing License in the State of Maryland.
  2. Bachelor's degree in Nursing or an equivalent field of study preferred.
  3. Current Cardiopulmonary Resuscitation Certificate (CPR)
