Key Responsibilities

  • Facilitate access to community resources and advocate for patients' rights and interests
  • Ensure timely and accurate documentation of case management activities
  • Monitor patient and program outcomes to identify areas for improvement
Requirements
  • Master's degree in social work (MSW) required
  • Minimum one year of clinical social work experience in a healthcare setting
  • Licensed Independent Social Worker (LISW) or Licensed Master Social Worker (LMSW) in the District of Columbia, Maryland, or Virginia
Preferred Qualifications
  • Experience with computer software programs in a Windows environment
  • Knowledge of community systems and resources in the Mid-Atlantic area
