This position involves a blend of office work and field engagement, making it an enriching and fulfilling role for a committed individual eager to make a difference in the community and in the lives of

participants needing permanent supportive housing. The working hours are from 9 AM to 5 PM, Monday through Friday.

Key Responsibilities:

Service Planning:

  • Develop comprehensive assessments to effectively engage participants regarding their unique strengths and experiences.

  • Utilize a client-centered approach to assist participants in crafting and executing an individualized action plan (IAP).

Housing Stability:

  • Aid participants in negotiations with landlords for necessary repairs and maintenance.

  • Coordinate inspections to ensure housing units are evaluated annually and as needed.

  • Build and sustain relationships with landlords to facilitate housing stability.

Financial Empowerment:

  • Assist participants in securing a sustainable income source, including employment and entitlements.

  • Educate and guide participants on accessing benefits and resources such as Social Security, EAEDC, SNAP/Food Stamps, and fuel assistance.

Health and Wellbeing:

  • Support participants in scheduling primary care and psychiatric/counseling appointments to promote health-related support.

  • Facilitate adherence to medical and psychiatric appointments by connecting participants with wrap-around services.

  • Provide transportation to participants to ensure they can meet their medical and social service needs.

  • Facilitate referrals to day programs and services to foster meaningful community connections.

Development and Additional Functions:

  • Attend required meetings, supervisions, and training sessions.

  • Complete documentation requirements, including comprehensive assessments and progress notes.

  • Communicate and document incidents promptly and accurately.

  • Perform other related duties as assigned or necessary.

Qualifications:

  • A valid driver's license, adequate automobile insurance, an acceptable driving record, and a reliable vehicle during working hours.

  • Proven ability to work independently, manage time effectively, prioritize tasks, take initiative, and collaborate as a team member.

  • Strong oral, written, computer, and organizational skills.

  • Ability to adapt to the evolving needs of program participants.

  • Excellent interpersonal skills, patience, compassion, and empathy.

  • Minimum age of eighteen years.
