About the Role
This is a full-time opportunity that requires a Registered Nurse (RN) with at least one year of relevant experience in a similar role. As a Senior Nursing Care Coordinator, you will be responsible for coordinating the care of patients across various settings, including hospitals, clinics, and home health services.
Responsibilities
- Develop and implement individualized care plans in collaboration with multidisciplinary teams to ensure seamless transitions and optimal patient outcomes.
- Conduct comprehensive assessments of patients' physical, emotional, and social needs, identifying opportunities for improvement and developing strategies to address them.
- Cultivate strong relationships with patients, families, and caregivers, providing education, support, and guidance throughout the care process.
- Collaborate with healthcare providers, social workers, and other stakeholders to ensure timely and effective communication and coordination of care.
- Maintain accurate and up-to-date records of patient information, including medical history, treatment plans, and progress notes.
Requirements
To succeed in this role, you will need:
- A current RN license in the appropriate state.
- At least one year of relevant experience as a Registered Nurse, preferably in a similar care coordination role.
- Excellent communication and interpersonal skills, with the ability to build trust and rapport with diverse populations.
- A keen understanding of healthcare regulations, policies, and procedures, with the ability to navigate complex systems and workflows.
