Overview:
A leading provider of integrated medical services seeks a compassionate nurse practitioner to deliver personalized care to seniors and patients with complex needs in various settings.

Salary: $105,000 - $120,000 per year, based on performance, with opportunities for quarterly salary increases.

Job Description:
As a house calls nurse practitioner at Abode Care Partners, you will provide evidence-based medical care by planning, directing, and evaluating patient medical needs. This includes examining and treating chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses. You will order and interpret laboratory and diagnostic testing as needed, collaborate with caregivers and other healthcare professionals in providing interdisciplinary care, and maintain a comprehensive plan of care. Additionally, you will ensure patient medical records are kept up-to-date and provide referrals to other healthcare practitioners when necessary.

Responsibilities:
• Performs routine health checks and conducts assessments of patients' medical conditions
• Develops and implements individualized care plans to meet the unique needs of each patient
• Coordinates with other healthcare professionals to ensure comprehensive and coordinated care
• Maintains accurate and detailed patient records, including medical history, test results, and treatment plans
• Communicates effectively with patients, families, and healthcare teams to ensure seamless care delivery

Qualifications:
A Master's degree in Nursing (MSN) is required, along with five or more years of nursing experience, preferably two years as a nurse practitioner. Active state NP licensure, national board certification, and DEA licensure are also necessary. Strong interpersonal communication skills, excellent time management, proficiency in medical instruments and equipment, and knowledge of HIPAA and OSHA regulations are essential. Ability to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day, is also required.
