We are seeking a highly skilled Care Coordinator to join our team at CAI. As a Nurse Case Manager, you will play a vital role in coordinating and implementing care management for members with complex health needs in Delaware.

About the Job

This is a full-time position that involves conducting assessments, creating care plans, and facilitating transitions between care settings while ensuring appropriate resources are available. The successful candidate will have strong communication skills, collaboration with healthcare providers, and adherence to state regulations.

The ideal candidate will be able to work in a hybrid environment and will have experience with case management, discharge planning, and telehealth. If you are a motivated and detail-oriented individual who is passionate about delivering high-quality patient care, we encourage you to apply.

Responsibilities
  • Conduct face-to-face assessments with members in their homes, nursing facilities, or other community-based settings
  • Develop and implement personalized care plans to meet the unique needs of each member
  • Facilitate transitions to alternate care settings, such as hospital to home or nursing facility to community setting
  • Educate members or caregivers regarding health care needs, available benefits, resources, and services
  • Provide education, resources, and assistance to help members achieve goals as outlined in their plan of care
  • Collaborate with healthcare providers, including PCPs, occupational therapists, and behavioral health professionals
  • Document all case management services and interventions in the electronic health record
Requirements
  • Current Registered Nurse licensure in the state of Delaware
  • At least 2 years of experience in case management and discharge planning
  • Experience working with complex populations, including geriatric special needs and behavioral health
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Proficiency in PC-based word processing and database documentation
  • Reliable transportation daily to travel within assigned territory
