Job SummaryWe're seeking a highly skilled and compassionate Nursing Care Specialist to join our team. As a key member of our care-focused environment, you'll work closely with our residents, their families, and our team members to provide individualized care and support. Your primary responsibilities will include:
- Conducting regular wellness visits for all residents, monitoring their health and well-being, and documenting any changes or concerns.
- Collaborating with our Resident Care Director to develop and implement personalized care plans that meet the unique needs of each resident.
- Maintaining accurate and up-to-date medical records, ensuring that all necessary documentation is completed and submitted in a timely manner.
- Providing clinical support and assistance to our community team members as needed, promoting a culture of teamwork and collaboration.
- Staying up-to-date with the latest developments in nursing practices, techniques, and methods, applying this knowledge to improve the quality of care we provide.
To be successful in this role, you'll need to have:
- A graduate degree in nursing from an accredited institution.
- A current state license as a professional Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN).
- A minimum of one year's experience in a home health, assisted living, hospital, or long-term care environment.
- Demonstrated knowledge of nursing practices, techniques, and methods applied to health and wellness resident care, as well as federal, state, and local regulations.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and team members.
- The ability to work independently and as part of a team, prioritizing tasks and managing time effectively in a fast-paced environment.
We offer a competitive salary range of $55,000 - $65,000 per year, depending on your level of experience and qualifications. Additionally, you'll receive a comprehensive benefits package, including medical, dental, and vision insurance, retirement savings plans, paid time off, and tuition reimbursement.
