Job Summary

We are seeking a skilled Senior Healthcare Coordinator RN to join our team at CAI. As a seasoned healthcare professional, you will be responsible for providing community-based care coordination and management for clients in Delaware.

This role involves assessing, planning, and implementing care for members with complex healthcare needs while facilitating transitions between care settings and educating members about available resources.

Candidates must possess a strong background in case management, nursing experience, and excellent communication skills.

About the Role

The ideal candidate will have:

  • A current Registered Nurse license in the state of Delaware
  • At least 2+ years of experience in Case Management and Discharge Planning
  • Experience working with diverse populations, including geriatric special needs, behavioral health, and home health
  • Excellent communication and interpersonal skills
  • Ability to work independently and collaboratively as part of a team
Responsibilities

The Senior Healthcare Coordinator RN will be responsible for:

  • Conducting face-to-face needs assessments with members and their caregivers
  • Developing and implementing individualized care plans to meet members' unique needs
  • Collaborating with healthcare providers, social workers, and other stakeholders to ensure seamless care coordination
  • Educating members and caregivers on available resources and services
  • Documenting all case management activities and interventions in the electronic health record
