Job Summary

We are seeking a highly skilled and compassionate Youth Outreach Specialist to join our team at Peoples Family of Corporations. As a key member of our outreach team, you will be responsible for providing intensive outreach and engagement services to youth and their caregivers in crisis response to mental health or substance abuse symptoms.

Key Responsibilities
  1. Client Assessments and Service Planning: Conduct thorough client assessments to develop personalized service plans that focus on the client's strengths and needs, working closely with the ERE Team to ensure plan implementation.
  2. Community Outreach and Engagement: Engage in routine home visits and non-traditional outreach efforts to develop rapport with clients and connect them with relevant services that meet their individual needs.
  3. Recovery-Oriented Care Planning: Collaborate with the ERE Team to develop recovery-oriented care plans that address behavioral health, medical, and social barriers for clients, with a focus on crisis response and stabilization.
  4. Resource Development and Referral: Build and maintain knowledge of community resources and effectively match clients with eligible services, ensuring timely and accurate communication with referral sources.
  5. Crisis Response and Stabilization: Provide intensive and creative attempts to help clients access necessary services, including advocacy and support to overcome barriers to service.
  6. Service Monitoring and Documentation: Maintain regular contact with clients and service providers to ensure seamless service delivery, accurately completing required documentation within designated timeframes.
  7. Client Support Network Development: Assist clients in expanding their support network outside of the behavioral health service delivery system.
  8. Team Collaboration and Training: Participate in weekly ERE Outreach Team meetings to strategize case management and engage in ongoing training opportunities to enhance project procedures, outreach/engagement techniques, and clinical interventions.
  9. Confidentiality and Compliance: Safeguard Protected Health Information and maintain responsiveness to Corporate Compliance and Quality Assurance procedures, adhering to established procedures, rules, and regulations of the clinical records department.
  10. Clinical Competencies: Demonstrate proficiency in trauma-informed care, stages of change, and motivational interviewing, as well as the ability to work with individuals with multiple challenges.
  11. Interdisciplinary Collaboration: Work collaboratively and respectfully with internal and external agency providers to ensure high-quality care and build healthy partnerships.
  12. Teamwork and Communication: Maintain positive work relationships, demonstrate cooperation with peers and supervisors, and contribute to a team environment.
Requirements
  1. Education and Training: Bachelor's degree in a social services field, Master's degree preferred (i.e., Social Work, Psychology, Counseling, or Psychiatric Nursing), with licensure preferred (or provisional license or licensure-eligible).
  2. Experience: Minimum of 2 years' experience providing services to youth with severe emotional disturbance, with CPR, First Aid, and Medication Administration certifications, a valid driver's license, and insurance.
  3. Knowledge and Skills: Knowledge of the Eastern Region Mental Health Network, community resources, entitlement programs, and the DSM-5, with the ability to think creatively and resolve crises, as well as possess good problem-solving skills.
