Job Description

Penndel Mental Health Center is seeking a highly motivated Support Coordinator to join our team. As a Support Coordinator, you will work with individuals diagnosed with mental illness, intellectual disability, or drug and alcohol addiction, providing Supports Coordination services and advocating for their needs.

Your primary responsibility will be to assess individual needs, develop Annual Plans, and coordinate services and supports. You will work closely with individuals, families, and other professionals to ensure that individuals receive the necessary support to live independently and achieve their goals.

Key responsibilities include:

  • Developing Annual Plans according to regulations and guidelines, in collaboration with individuals and families.
  • Coordinating services and supports, including financial, transportation, recreation, respite, adaptive equipment, home adaptation services, behavioral health, medical resources, and other supports as needed.
  • Acting as an advocate for individuals in the community and with public and private entities.
  • Monitoring services funded through the OMR system in accordance with funding stream regulations.

This position offers a competitive salary range of $70,000 - $80,000 per year, based on experience, and a productivity bonus. We also provide very competitive medical, dental, and vision insurance coverage, opportunities for continuing education, and vacation, sick, and personal time.
