Job Title: Co-Occurring Disorder Specialist

About the Role:

The Co-Occurring Disorder Specialist will provide treatment services to promote recovery and community integration for individuals served on the Assertive Community Treatment (ACT) team. This role will utilize an Integrated Dual Disorder (IDDT) model of care to address the complex needs of individuals with co-occurring mental health and substance use disorders.

Key Responsibilities:

  • Conduct drug and alcohol assessments and provide stage-based individual and group mental health/substance use interventions.
  • Provide cross-training and guidance to the team on recommended practices to support engagement, active treatment, and maintenance of recovery.
  • Help individuals access and maintain benefits and community resources.
  • Provide care coordination with external providers and connect with natural supports.
  • Offer crisis intervention services and participate in on-call rotations.
  • Collaborate with the ACT team to develop comprehensive assessments, individualized treatment plans, and substance abuse treatment.
  • Lead the team in assessing, planning, and treating substance abuse for assigned clients.
  • Facilitate admissions to inpatient CD treatment and linkage to community resources for outpatient support.
  • Provide coaching and education on independent living skills, including cooking, shopping, cleaning, personal hygiene, transportation, budgeting, organization, and housing.
  • Collaborate with the Vocational Specialist to encourage competitive employment and assist in fostering positive work skills.
  • Collaborate with the ACT nurse and healthcare providers to promote physical and dental wellness goals.
  • Attend commitment and other court hearings as required, complete required court reports, and testify when requested.
  • File reports with Vulnerable Adult, Child Protection, the Ombudsman's Office, and other mandated/required reports when applicable.

Requirements:

  • Bachelor's or Master's Degree in Behavioral Sciences or a related field.
  • 2,000 hours of supervised experience working with individuals with mental illness.
  • 2,000 hours of training or supervised experience in substance abuse treatment.
