At Radias Health, we are committed to providing high-quality services to individuals experiencing homelessness. We are seeking a skilled and compassionate Homeless Outreach Mental Health Practitioner to join our team.

Job Summary

The Homeless Outreach Mental Health Practitioner will provide professional services to individuals experiencing homelessness, focusing on mental health and wellness. This role requires a strong understanding of the complexities of homelessness and the ability to work effectively with individuals in crisis.

Key Responsibilities
  • Provide outreach services to individuals experiencing homelessness, including screening, case management, and re-engagement.
  • Develop and implement recovery and housing plans in collaboration with clients.
  • Maintain an active caseload of 15-20 clients and provide regular check-ins and support.
  • Work collaboratively with a multidisciplinary team, including an Advanced Nurse Practitioner, Mental Health Professionals, and Certified Peer Specialists.
  • Identify candidates for Section 811 and PHOP subsidy programs and facilitate the application process.
  • Engage in coordinated outreach efforts with RADIAS Health outreach staff and other community outreach providers.
  • Coordinate cases with other county or community agencies, including Intake, Mental Health Case Management, Adult Protection, Income Maintenance, Crisis Center, and community hospitals.
  • Develop and implement care plans with enrolled clients and provide daily living support as needed.
  • Assess and respond to crisis situations, including referrals to emergency psychiatric or other crisis intervention services as needed.
  • Provide education and support to clients regarding mental health, wellness, and self-advocacy.
  • Participate in regular case consult meetings and attend required staff meetings and trainings.
  • Transport clients in the community as needed and maintain a clean driving record.
Requirements
  • 6,000 hours of supervised experience in the delivery of services to individuals with mental illness.
  • Experience supporting individuals experiencing homelessness and knowledge of services/resources in Ramsey County.
  • Current and valid driver's license with current and valid auto insurance.
Preferred Qualifications
  • Bachelor's degree in behavioral sciences or related fields.
  • Experience providing outreach services to individuals experiencing homelessness and knowledge of services/resources in Ramsey County.
