About the Role

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Sacred Journey Hospice team. As an LPN, you will play a vital role in providing high-quality patient care and support to individuals and their families during their end-of-life journey.

Responsibilities
  • Provide direct patient care, including pain and symptom management, to ensure optimal comfort and well-being.
  • Implement nursing practice based on comprehensive assessments and care plans, ensuring patient-centered care.
  • Document patient services accurately and timely, maintaining clear and concise records.
  • Assess and educate patients and their families/caregivers on disease management, treatment options, and self-care techniques.
  • Participate in the coordination of hospice services, collaborating with the interdisciplinary team to ensure seamless care.
  • Manage pain and symptoms effectively, evaluating patient response to treatments and medications.
  • Engage in professional growth and development opportunities, staying up-to-date on best practices and quality assessment activities.
  • Ensure compliance with state and federal regulations, maintaining the highest standards of care.
Requirements
  • Graduate of an accredited practical or vocational nursing program, with current licensure to practice in the state of operation.
  • At least one year of nursing experience, with three or more years preferred.
  • Experience in community health, hospice, or med/surg settings is highly desirable.
  • Knowledge of the hospice philosophy of care and commitment to clinical and documentation excellence.
