About the Role

We are seeking a highly skilled and detail-oriented Population Health Coordinator to join our team at Advocate Aurora Health. As a key member of our population health management team, you will play a critical role in coordinating care for our patients and ensuring seamless transitions between healthcare services.

Key Responsibilities
  • Coordinate intake of cases on new and established assignments to expedite the care coordination process
  • Facilitate medical review processes, verify health plan eligibility, and obtain necessary data
  • Research, organize, and maintain resources, guidelines, and policies for the team
  • Implement new employer groups and ensure adequate onboarding materials are available
  • Coordinate care gap outreach and utilize established criteria to generate lists of members requiring intervention
Requirements
  • Typically requires 5 years of experience in population health management, utilization management, claims, health insurance processing, clinical or care management/quality environment
  • Proficiency in ICD-10, CPT, and HCPCS coding preferred
  • Knowledge of medical terminology and ability to manage data systemically
  • Excellent organizational, interpersonal, and communication skills
