We are seeking a skilled Intake Coordinator to join our Home Health team. As a key member of our team, you will be responsible for coordinating the intake of new patients and planning for future agency services.
Key Responsibilities- Coordinate the intake of new patients to Home Health, ensuring a seamless transition into our care.
- Collaborate with referral sources, physicians, and patients/families to plan for future agency services.
- Seek out necessary information from appropriate resources to complete the intake process.
- High School Diploma or equivalent required.
- Home Health/Hospice experience preferred.
- Customer service experience interacting with patients strongly preferred.
- Referral experience preferred.
