**Job Summary**

Elliott Health is seeking a highly skilled Social Worker Case Manager to join our Care Coordination Department. As a key member of our team, you will provide clinical social work interventions with patients and families to address personal and environmental issues, maximizing emotional, social, and physical well-being.

**Key Responsibilities:**

  • Provide systematic approach to evaluation of patient care
  • Coordinate patient care through the continuum, facilitating optimal quality outcomes in relation to clinical care and cost effectiveness
  • Collaborate with the primary nurse and multidisciplinary team in ongoing clinical social work assessment of patient's status/care management
  • Assist in development, implementation, and evaluation of integrated clinical pathways
  • Monitor patient's progress to ensure care is appropriate and timely

**Requirements:**

  • Master of Science in Social Work required
  • One (1) to three (3) years of experience in a healthcare system, preferably in a hospital setting
