Social Work Program Coordinator is assigned to the Primary Care Mental Health Integration program- an interdisciplinary treatment team which provides co-located mental health services in the Primary Care (PC), Urgent Care (UC), Community Based Outpatient Clinics (CBOCs), and other clinic locations, as assigned.

Employee also works closely with a diverse group of Veterans and their support persons to improve clinical and programmatic outcomes.


FUNCTIONS OR SCOPE OF ASSIGNED DUTIES
Clinical Responsibilities

  • Conducts biopsychosocial assessments and formulates and implements short-term
treatment plans for individuals, groups, couples and families from various socioeconomic,
cultural, ethnic, educational and other diversified backgrounds who are
experiencing a variety of psychiatric, substance use, medical and social problems.

  • Completes functional assessments and utilizes brief, focused therapeutic interventions to
help Veterans meet their treatment goals in the PC setting. Best practices, practice
guidelines, and evidence-based treatment protocols are utilized to the extent feasible in
all therapeutic interventions.

  • Responsible to ensure continuity of care for all Veterans presenting in PC for mental
health, by providing intervention or facilitating services with another provider. Social
Work Program Coordinator will be responsible for follow-up care management to ensure
that Veterans are engaged in care as indicated in the short-term treatment plan.

  • Ensures productivity is maintained at the established level for the program to which
Social Work Program Coordinator is assigned.

  • Complies with national and local performance measures, including timely health care
screening and completion of applicable clinical reminders and encounter documentation.

  • Develops and documents treatment plans per current requirements.
  • Provides services to Veterans seen in PC or other clinics in-person and through the use of
face-to-face care and telehealth technology: VA Video Connect (VVC) and Clinical
Video Telehealth (CVT).

  • Evaluates Veterans in crisis for involuntary psychiatric commitment and completes
appropriate paperwork for involuntary psychiatric commitment when necessary. Social
Work Program Coordinator serves as a resource to Manchester VA staff regarding the
local laws and regulations regarding involuntary psychiatric commitment.

  • Assess suicide/homicide risk factors using standardized and validated suicide screening
and risk measures, identify protective factors, and clearly interpret the safety and
recommended level of care for the Veteran. Social Work Program Coordinator completes
suicide safety plans and Suicide Behavior/Overdose Reports in accordance with policy.
SWPC collaborates with the Suicide Prevention program for Veterans assessed to be at
increased risk for suicide.

  • Documents psychological interventions and activities in Veteran's clinical record
utilizing the electronic medical chart (CPRS) and ensures an appropriate hand-off and
documentation when transferring Veteran care. Social Work Program Coordinator will
track clinical contacts to ensure continuity of care.

  • Participates in the mandatory Mental Health on-call rotation.
Program Coordination/Administrative Responsibilities

  • Serves as the liaison between Mental Health and Primary Care departments.
  • Serves as the identified MH Same Day Access Champion. Employee will maintain and
update documentation and dissemination of MH Same Day Access services guidelines,
processes, and procedures. They will ensure compliance with access to same-day MH
services.

  • Serves as the facility Measurement Based Care Champion to ensure effective
implementation and maintenance per national guidelines.

  • The SWPC will serve as the assigned PCMHI Local Champion and clinical trainer.
  • The SWPC will serve as Manchester VAMC POC for VISN1 Primary Care Mental
Health Integration committees and workgroups, as assigned.

  • Works with MH leadership to ensure efficient operation of PCMHI programming,
program monitoring, outcome measures, and quality assurance.

  • Responsible for daily PCMHI program operations and integration with PACTs. Provides
day-to-day oversight of PCMHI employees

  • Responsible for timely completion of all program evaluations and documents, as
appropriate.

  • May serve on Medical Center or healthcare system, VISN and/or National VA
committees or task forces, as assigned.

  • At the direction of management, employee represents the Medical Center or healthcare
system at professional/program community group/events and committees in
conferences/meetings with higher-ranking managers, supervisors and staff members from
other disciplines or work units and/or with representatives of local public interest groups.
