We are seeking a highly skilled and compassionate Oncology Social Worker to join our team at Elliot Health. As a key member of our multidisciplinary team, you will play a vital role in providing exceptional patient care and support.
Key Responsibilities- Collaborate with the primary nurse and multidisciplinary team to conduct comprehensive clinical social work assessments of patients.
- Identify and connect patients with relevant community resources to ensure seamless care transitions.
- Provide supportive counseling and crisis intervention to patients and their families as needed.
- Coordinate with community health facilities to ensure patient needs are met.
- Refer patients to homecare, acute rehab, and nursing home services as required.
- Serve as a liaison between community social service agencies and patients to address identified needs.
- Master of Science in Social Work (MSW) degree required.
- 1-3 years of experience working with Oncology patients and cancer survivors preferred.
- Excellent clinical documentation and EMR system skills preferred.
