We are seeking a skilled Social Worker to join our team at SolutionHealth, a leading provider of in-home healthcare services in Manchester and Southern New Hampshire. As a Visiting Nurse Association Social Worker, you will play a vital role in delivering high-quality care to our patients and their families.
Key Responsibilities- Provide social work interventions to active clients/patients and family members, assessing their psychosocial, environmental, financial, and functional status to coordinate appropriate plans.
- Document social work interventions in the client's/patient's medical record, including referrals, issues, recommendations, and treatment plans.
- Assist clients/patients and their families in coping with residual physical disabilities and fear related to helplessness and death through clinical social work intervention.
- Coordinate family meetings as needed and participate in case conferences with other healthcare professionals to develop and implement plans of care.
- Travel to client/patient homes, meetings, and conferences, which are held off-site.
- Master of Science in Social Work required.
- One (1) to three (3) years of relevant professional experience in a healthcare setting.
- Valid Driver's License and proof of automobile insurance required.
