Home Health Case Manager RN - Specialty Infusions

Location: Northwest Suburbs

Hours: Days with Wkd and On Call Rotation

This position is eligible for a sign-on bonus. You must be working in this same position at the time of payout. Current internal team members are not eligible at this time.

Key Responsibilities:

  • Patient Outcomes:
    • Conduct holistic assessments on assigned patients according to established procedures.
    • Develop and adhere to visit frequencies for patient plans of care.
    • Assess changes in patient conditions that could be potentially hazardous or life-threatening.
    • Establish and update patient plans of care every 60 days and as needed.
    • Implement and revise patient care plans accordingly.
    • Develop discharge plans for patients and significant others in a timely manner.
    • Identify interventions to achieve desired patient outcomes.
    • Evaluate patient outcomes in response to actions of the healthcare team.
  • Patient Education:
    • Minimize patient and family knowledge deficits regarding disease processes, patient care, medications, infection control, and specific procedures.
    • Provide anticipatory guidance and teaching to patients and caregivers to optimize quality of life and prevent unnecessary negative outcomes.
    • Document teaching strategies with patients, families, and significant others.
    • Participate in inservices and orientation of new employees.
    • Provide preceptorship for new staff nurses learning infusion therapy techniques.
  • Human Relations:
    • Utilize therapeutic communication skills and effective interpersonal skills with patients, caregivers, and significant others.
    • Promote multidisciplinary coordination and conduct multidisciplinary conferences at least once during each patient's round of care.
    • Participate in creating a cooperative working environment.
    • Maintain confidentiality of information in the field and office setting.
    • Communicate patient care strategies and patient problems/needs to relevant coworkers.
  • Agency and Departmental Goals:
    • Demonstrate commitment and actively support department and organization/agency goals.
    • Maintain and support all regulatory agency requirements.
    • Attend to personal work schedule in a consistent and professional manner according to established agency policies/procedures.
    • Demonstrate cost-effective measures in nursing practice.
    • Utilize basic safety measures and principles.
  • Professional Growth:
    • Maintain current infusion therapy skills and knowledge.
    • Establish and achieve self-development objectives.
    • Demonstrate self-direction for own continuing education.
    • Practice new techniques and practices as implemented by nursing administration.
  • Productivity and Revenue Generation:
    • Maintain expected productivity.
    • Adhere to determined frequency of visits.

Licensure:

  • Nurse, Registered (RN)

Education/Experience Required:

  • Graduate from an accredited school of nursing
  • Minimum 2 years Medical/Surgical experience
  • Critical care experience a plus

Knowledge, Skills & Abilities Required:

  • Good communication and interpersonal skills
  • Good documentation skills
  • Good problem-solving skills
  • Ability to determine appropriate frequency of visits
  • CPR
  • Current licensure in the State of Illinois as a registered nurse
  • CRNI preferred

Physical Requirements and Working Conditions:

  • Extensive traveling within geographical boundaries in all extremes of weather
  • Ability to transport patients is required of this position
  • Exposed to hazardous materials with respect to patient care
  • Exposed to widely varying social and environmental conditions in the patient's neighborhoods and homes
  • Should have some means of insured transportation
  • Involves high travel and large territory coverage
  • On-call responsibilities may be frequent

COMPENSATION: $74,672.00-$112,112.00 ANNUAL BASE SALARY
