Job Summary

We are seeking a highly skilled and compassionate Case Manager to join our team at Little City Foundation. As a Case Manager, you will be responsible for providing direct and indirect support to adults with developmental disabilities, ensuring their dignity, self-advocacy, and increased autonomy.

Key Responsibilities
  • Carry an average caseload of up to 40 adults receiving residential services in various living arrangements.
  • Serve as primary service provider advocate to participants, promoting self-advocacy and ensuring individual participation in all decisions affecting their life.
  • Monitor the health, safety, and well-being of each person served, conducting on-site visits to short-term rehabilitation facilities and hospitals as needed.
  • Transport residents to/from medical appointments and advocate on their behalf.
  • Collaborate with the interdisciplinary team to develop and implement person-centered plans, ensuring all team members are invited to meetings and that implementation strategies are completed within 20 days.
  • Complete mental health assessments, quarterly and six-month reports, and ensure required paperwork is complete and provided.
  • Maintain accountability for participant files and ensure delivery and/or coordination of all residential services are in compliance with policies and procedures.
Requirements
  • Bachelor's degree in a human services field, including sociology, special education, rehabilitation counseling, and psychology.
  • At least one year of experience working directly with people with developmental disabilities, meeting the minimum federal educational requirements for a QIDP outlined in 42 CFR.
  • Excellent interpersonal skills, strong verbal and written communication skills, and good basic computer skills.
  • Strong organizational skills to manage multiple priorities, capable of transporting adults, and a valid driver's license with a good driving record.
Work Environment

The physical demands of this job include sitting, walking, and standing for prolonged periods, frequently pushing, pulling, grasping, feeling, and occasionally stooping, kneeling, crouching, or crawling. The employee is subject to assist and lift up to 50 pounds with varying amounts of assistance on a reoccurring basis.

The employee is required to have corrected vision and hearing within normal range and the ability to operate a motor vehicle. Occupational exposure to blood, body fluids, infectious substances, including communicable diseases when performing personal care (Category III). Occasional exposure to cleaning agents and disinfectants. Residential work environment with a noise level of normal to loud.
