Lead Housing Case Manager

Job Summary:
The Lead Housing Case Manager plays a critical role in supporting Plymouth residents by working as part of an onsite permanent supportive housing (PSH) team. This team is committed to delivering high-quality services based on the values inherent in Plymouth's mission, the principles of cultural proficiency, the tenets of harm reduction and trauma-informed care, and the 'housing first' philosophy.
Key Responsibilities:
- Provides mentorship, coaching, support, and guidance to Housing Case Managers by assisting with onboarding, training, and guidance on case management techniques, access to resources, and establishing positive resident relationships.
- Assists the Residential Services Manager in coordinating daily operations, ensuring continuity of services, and implementing policies and procedures.
- Leads regular 1:1 or team peer learning sessions to support quality service delivery, program compliance, and ongoing learning.
- Supports the Residential Services Manager in aligning tenant service strategies with Residential Specialists, Community Specialists, and other tenant-facing staff.
- Works with Property Management to support tenant recertification, as needed.
- Ensures continuity of function and service in the event of an HCM absence.
- Rotates with the Lead Residential Specialist to serve as backup to the Residential Services Manager during their absence.
Supportive Services:
- Develops and maintains Housing Service Plans with residents, outlining goals to address transition needs and long-term housing stability needs.
- Maintains extensive knowledge of resources available to residents, including public benefits and community organizations.
- Assists residents in scheduling appointments, arranging transportation, and accompanying them to appointments as needed.
- Advocates, consults, and collaborates with community providers to ensure continuity of care.
- Mediates conflict using de-escalation and other techniques.
- Intervenes in crises, responds to emergencies, and initiates action as required, focusing on harm reduction and trauma-informed care.
Collaborative and Community Building:
- Attends and participates in scheduled supervision and team meetings to discuss resident issues and general program issues and goals.
- Practices a cooperative work approach and promotes teamwork among co-workers and community partners.
- Collaborates with other Plymouth Housing Departments and participates in committees or workgroups as appropriate.
Qualitative and Administrative:
- Works to the standard and requirements of the service program agreements, including program goals relating to residential stability, eviction prevention, enhanced income/skills, and self-determination.
- Maintains all database records and other required documentation in an accurate and timely manner, following strict confidentiality to protect resident data.
- Ensures significant events involving residents and building operations activities are recorded accurately in the daily log.
Requirements:
- BA/BS degree in a human service field or equivalent experience.
- Minimum three years of experience working with homeless and low-income individuals, including persons living with mental and physical illnesses, chemical dependency/addictions, disabilities, and HIV/AIDS.
- Minimum two years working as a case manager, with at least one year working with individuals experiencing homelessness.
- Valid Washington State Driver License and insurable driving record.
Desired Qualifications:
- Experience working with diverse, low-income, or special needs populations.
- Direct coaching or mentoring experience.
