Behavioral Health Case Manager

About the Role:
Plymouth Housing Group is seeking a highly skilled Behavioral Health Case Manager to join our team. As a Behavioral Health Case Manager, you will play a critical role in providing comprehensive support to resident populations with the goal of lessening behavioral health inequities and improving wellbeing.
Key Responsibilities:
- Collaborate with residents, PSH Case Managers, and team members to assess behavioral health needs and develop care plans.
- Facilitate access to behavioral health, substance use, and medical treatments.
- Connect residents with internal and external clinical resources.
- Administer and follow-up on symptom screenings.
- Conduct ongoing assessments to monitor residents' health status and treatment progress.
- Provide support and advocacy for residents navigating the healthcare system.
- Maintain accurate documentation of interactions and interventions.
Requirements:
- Master's degree from an accredited university or college, preferably in social work, or 3-5 years of professional and leadership experience in the relevant program area.
- Active associate-level clinical licensure in Washington State in one or more of the following:
- Licensed Family Counselor;
- Marriage and Family Therapist Associate License;
- Mental Health Counselor Associate License;
- Or professional license/certificate in the relevant program area.
- Experience working with individuals experiencing behavioral health challenges, substance use disorders, and homelessness.
- Knowledge of trauma-informed care, harm reduction, and evidence-based practices in behavioral health.
- Strong communication and collaboration skills.
- Ability to work effectively in a multidisciplinary team environment.
- Commitment to cultural humility, equity, and social justice.
