Job Summary

The Sophia Way is seeking a dedicated and compassionate Housing Case Manager to join our Programs team. As a Housing Case Manager, you will play a pivotal role in helping individuals experiencing homelessness achieve housing stability, employment, and access to vital community resources.

Key Responsibilities
  • Manage a caseload of up to 40 clients, providing case management services in the Project-Based Section 8, shelter, and transitional housing programs.
  • Develop Housing Stability Plans and service plans tailored to clients' individual needs in areas such as mental health, recovery, healthcare, education, and employment.
  • Engage clients in building life skills and provide supportive services including safety assessments, housing advocacy, and connections to community resources.
  • Assist with housing applications, state benefits, and essential documentation for clients.
  • Conduct regular progress evaluations with clients, meet weekly with shelter clients, and work two shifts per month in shelter operations.
  • Lead workshops at the Day Center, contribute to new program development, and collaborate with the Client Services team on cross-functional projects.
  • Maintain detailed, confidential client records in the HMIS system, ensuring compliance with all internal and external regulations.
Qualifications
  • Bachelor's degree in social or human services, psychology, sociology, or related field, or equivalent combination of education and experience.
  • At least two years of professional experience working with homeless populations, mental health, and substance abuse.
  • Strong understanding of King County's Section 8 program, local housing resources, and community services.
  • Experience working with diverse populations, with cultural and social sensitivity.
  • Strong organizational skills and the ability to manage multiple tasks, prioritize effectively, and work independently.
  • Proficiency in standard office applications; HMIS experience is a plus.
  • Excellent verbal and written communication skills, with the ability to engage various stakeholders.
  • Flexibility for occasional weekend, evening, and holiday shifts and regional travel within King County.
Certifications
  • CPR/First Aid, Bloodborne Pathogens, and recent TB test (or ability to obtain upon hire).
  • Valid driver's license and food handler's permit (or ability to obtain upon hire).
