Exceptional Hospice RN Case Manager Opportunity

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice RN Case Manager. As a key member of our interdisciplinary team, you will provide exceptional care to patients and families in need of hospice services.

Key Responsibilities:
  • Maintain a specified case management load, ensuring timely and effective care coordination.
  • Conduct comprehensive assessments of patients and families to determine hospice needs, including pain and symptom management.
  • Participate in on-call duties as defined by our office on-call policy.
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
  • Coordinate care among IDG members and other healthcare providers to ensure seamless delivery of services.
  • Evaluate and coordinate the need for services, including inpatient, respite, and continuous care.
  • Supervise and direct the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients and families.
Requirements:
  • Graduate of an accredited school of nursing.
  • Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
  • Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
  • One (1) year of hospice care experience, with precepting or training experience preferred.
  • CPR certification and valid driver's license with auto liability insurance.
