At Interim HealthCare Staffing, we are seeking a compassionate and skilled Hospice RN Case Manager to join our team. As a Hospice RN Case Manager, you will play a vital role in providing exceptional care to patients and families in need.
Key Responsibilities:- Maintain a specified case management load, ensuring timely and effective care delivery.
- Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
- Collaborate with the Interdisciplinary Group (IDG) to ensure seamless care coordination and implementation of care plans.
- Direct and supervise the services provided by LPN/LVN, Hospice Aide, and Homemaker assigned to patients and families.
- Evaluate and coordinate the need for services, ensuring the continuous assessment of patient and family needs.
- Facilitate the consideration of alternative levels of care when indicated by patient and family status and needs.
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina, with an active and good-standing license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
- One (1) year of hospice care experience.
- One (1) year of precepting or training experience preferred.
- CPR certification.
- Able to provide proof of valid driver's license and auto liability insurance.
- Competitive Pay $80K to $86K DOE.
