Key Responsibilities:- Maintain a specified case management load to ensure timely and effective care delivery.
- Conduct comprehensive assessments of patients and families to determine their unique needs and develop personalized care plans.
- Collaborate with healthcare providers to coordinate care and ensure seamless transitions.
- Evaluate and coordinate the need for services to support patients and families.
- Supervise and mentor LPN/LVN, Hospice Aide, and Homemaker staff to ensure high-quality care delivery.
- Facilitate the consideration of alternative care levels when indicated by patient or family status/needs.
- Graduate of an accredited school of nursing.
- Active RN license in South Carolina.
- 12 months of accumulated experience as a registered nurse within the last 5 years or evidence of current clinical knowledge.
- 1 year of hospice care experience.
- CPR certification.
- Valid driver's license and auto liability insurance.
- Competitive Pay $80K to $86K DOE.
