Hospice RN Case Manager

Job Summary:
We are seeking a compassionate and skilled Hospice RN Case Manager to join our team at Interim HealthCare Staffing. As a Hospice RN Case Manager, you will be responsible for providing exceptional hospice care to patients and families, upholding our high standards of nursing practice.
Key Responsibilities:
- Maintain a specified case management load and complete initial and ongoing assessments of patients' physical and psychosocial status.
- Develop and implement plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and families.
- Coordinate care among IDG members and other healthcare providers to ensure the continuous assessment of patients' needs and implementation of the IDG plan of care.
- Direct and supervise services provided by LPNs, Hospice Aides, and Homemakers assigned to patients.
- Evaluate and coordinate the need for services provided to patients in accordance with the IDG plan of care.
- Facilitate the consideration of another level of care when indicated by patients' status or needs.
Requirements:
- Graduate of an accredited school of nursing.
- Licensed to practice as a RN in the state of South Carolina with an active and good-standing license.
- Twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or evidence of current clinical knowledge.
- One (1) year of hospice care experience.
- One (1) year of precepting or training experience preferred.
- CPR certification.
- Able to provide proof of valid driver's license and auto liability insurance.
