We are in search of a Full-Time Case Manager who will be instrumental in providing community support and coordinating services for individuals facing mental health challenges. The Case Manager will assist clients in establishing and achieving their short-term and long-term objectives while connecting them with essential community resources. Upholding the dignity and rights of our clients is paramount, ensuring that services are delivered with respect and care.

The Case Manager will work closely with clients to develop and implement detailed care coordination and treatment plans. Through proactive advocacy, assessment, referral, and crisis intervention, the Case Manager will aid clients in managing their mental and physical health symptoms. The level of support provided will be tailored to meet the unique needs of each client.

Key Responsibilities:

  • Evaluate client strengths and needs, formulate individualized care plans, and regularly monitor progress.
  • Maintain consistent communication with clients, offering necessary support and facilitating crisis intervention as required.
  • Refer clients to additional services and resources, ensuring they receive the necessary assistance.
  • Collaborate with supervisors, team members, and external organizations to deliver effective and efficient services.
  • Assist in discharge planning and coordinate services for clients who have been hospitalized.
  • Support clients in scheduling and attending appointments as needed.
  • Complete all required documentation accurately and promptly.
  • Adhere to agency policies and procedures, engage in staff development activities, and support agency-wide initiatives.

Qualifications:

  1. High School Diploma or GED required, with experience in mental health; or
  2. Preferred: Bachelor's degree in social work or a related field, or three years of experience in behavioral health.
  3. CDCA certification is mandatory for Substance Use Disorder (SUD).
  4. A valid driver's license and proof of auto insurance are required.
