MDS Coordinator

We are seeking a highly skilled MDS Coordinator to join our team at Outfield Healthcare Partners. As an MDS Coordinator, you will play a critical role in ensuring the accuracy and completeness of resident assessments and care plans.
Key Responsibilities- Interdisciplinary Team Collaboration: Ensure that the interdisciplinary team makes informed decisions regarding the completion or non-completion of additional MDS assessments based on clinical criteria.
- Assessment Reference Date Management: Assist with the coordination and management of the daily stand-up meeting, including the review of resident care and the setting of assessment reference dates.
- Regulatory Compliance: Comply with federal and state regulations regarding the completion and coordination of the RAI process.
- MDS Documentation: Monitor MDS and care plan documentation for all residents, ensuring that documentation is present in the medical record to support MDS coding.
- Data Entry and Coding: Maintain current MDS status of assigned residents according to state and federal guidelines, and complete accurate coding of the MDS with information obtained via medical record review, observation, and interview with facility staff, residents, and family members.
- Interdisciplinary Team Meetings: Attend interdisciplinary team meetings, quality assurance meetings, and other meetings to gather information, communicate changes, and maintain and update records.
- Change Management: Assist the DON or designee with the identification of significant changes, physician orders, and verbal reports to ensure that the MDS and care plan are reflective of those changes.
- Communication and Scheduling: Prepare scheduling, notice of resident care planning conferences, and assist the DON in communication of outcomes/problems to responsible staff, residents, and/or responsible parties.
- Professional Development: Continuously update knowledge base related to data entry and computer technology.
- Electronic Submission: Complete electronic submission of required documentation to the state database and other entities per company policy.
- Validation and Correction: Ensure timely submission of the MDSs to the State with proper follow-up on validation errors, and maintain validation records from the submission process in a systematic and orderly fashion.
- Regulatory Surveys: Actively participate in the regulatory or certification survey process and the correction of deficiencies.
- Trend Analysis: Report trends from completed audits to the Quality Assurance Committee.
- RAI Process Management: Ensure the completion and timeliness of the RAI process from the MDS through the completion of the plan of care.
- RAI Process Tracking: Initiate and monitor RAI process tracking, discharge/reentry, and Medicaid tracking forms through the Point Click Care system.
