We are seeking a highly skilled MDS Coordinator to join our team at Outfield Healthcare Partners. As a key member of our healthcare operations team, you will be responsible for ensuring the accuracy and completeness of resident assessments and care plans.

Key Responsibilities
  • Ensure timely and accurate completion of MDS assessments and care plans
  • Coordinate and manage the daily stand-up meeting to review resident care and set assessment reference dates
  • Comply with federal and state regulations regarding RAI process completion and coordination
  • Monitor MDS and care plan documentation for all residents and ensure documentation is present in the medical record to support MDS coding
  • Maintain current MDS status of assigned residents according to state and federal guidelines
  • Complete accurate coding of the MDS with information obtained via medical record review, observation, and interview with facility staff, residents, and family members
  • Attend interdisciplinary team meetings, quality assurance meetings, and other meetings to gather information, communicate changes, and maintain and update records
  • Assist the DON or designee with identification of significant changes, physician orders, and verbal reports to ensure the MDS and care plan are reflective of those changes
  • Prepare scheduling, notice of resident care planning conferences, and assist the DON in communication of outcomes/problems to the responsible staff, resident, and/or responsible party
  • Continually update knowledge base related to data entry and computer technology
  • Complete electronic submission of required documentation to the state database and other entities per company policy
  • Ensure timely submission of the MDSs to the State with proper follow-up on validation errors and maintain validation records from the submission process in a systematic and orderly fashion
  • Actively participate in the regulatory or certification survey process and the correction of deficiencies
  • Report trends from completed audits to the Quality Assurance Committee
  • Assure the completion and timeliness of the RAI Process from the MDS through the completion of the plan of care
  • Initiate and monitor RAI process tracking, discharge/reentry, and Medicaid tracking forms through the Point Click Care system
