Position Overview

Role Summary: The Client Care Coordinator will deliver comprehensive, time-sensitive support services for individuals facing challenges due to their criminal backgrounds. This role involves conducting thorough assessments, assisting with employment opportunities, advocating for legal rights, and facilitating access to essential community resources.

Key Responsibilities:

  • Engage with clients through proactive outreach to identify those eligible for re-entry support.
  • Provide transportation for clients transitioning from correctional facilities.
  • Perform initial evaluations and ongoing assessments to monitor client progress.
  • Conduct regular case management sessions to offer direct support and guidance.
  • Evaluate client circumstances, strengths, and support systems to formulate personalized improvement plans.
  • Assist clients in overcoming obstacles related to their criminal history, including housing instability, employment challenges, and legal issues.
  • Connect clients with vital community services such as housing assistance, job training, healthcare, and mental health support.
  • Follow up with clients to track their progress and ensure positive outcomes.
  • Document client milestones, including job placements and skill development, while maintaining accurate records.
  • Adhere to reporting and eligibility standards for re-entry initiatives.
  • Collaborate with other programs to enhance service delivery for clients.
  • Perform additional duties as assigned.
  • Work primarily from a designated wellness facility, focusing on crisis intervention and prevention strategies.

Qualifications:

  • A minimum of a Bachelor's Degree in Criminal Justice, Human Services, Psychology, Sociology, or a related discipline.
  • A Master's Degree is preferred or at least two years of relevant experience working with high-risk populations.
  • Experience in case management, counseling, or therapeutic roles is advantageous.
  • A strong interest in social justice, legal advocacy, and Fair Chance Hiring initiatives.
  • A commitment to delivering high-quality programs and utilizing data for program evaluation.
  • Excellent written and verbal communication skills, with the ability to engage effectively with diverse individuals.
  • Experience working with individuals facing mental health challenges and complex life situations.
  • Self-motivated with the ability to work independently while collaborating with various stakeholders.
  • Fluency in Spanish is required.

Employment Type: Full-time

Compensation: $36,000 - $38,000 per year
