Job Description

At Choice Of New Rochelle In, we are dedicated to providing exceptional case management and advocacy services for individuals with mental health or physical health illnesses. We are seeking a highly skilled and compassionate Client Care Coordinator to join our team.

Key Responsibilities:
  • Client Goal Setting: Work with clients to establish and achieve their mental health goals, such as securing housing, accessing education and employment opportunities, and connecting with primary care providers.
  • Care Activities: Engage in activities that empower clients to achieve their goals, such as providing emotional support, connecting clients with community resources, and advocating on their behalf.
  • Data Management: Utilize case management systems to collect and enter data on client progress, ensuring accurate and timely reporting.
  • Client Advocacy: Act as a voice for clients in navigating complex systems, such as social services and healthcare, to ensure their rights are protected and their needs are met.
Requirements:
  • STAR Qualities: Possess Solution-Oriented, Tenacious, Above & Beyond, and Responsible qualities, with a strong ability to balance objectivity with empathy.
  • Experience: Minimum 1 year of experience in case management, mental health, or a related field.
  • Education: Bachelor's degree in a related field, such as social work, psychology, or counseling.
