Job Summary

As a Homeless Outreach Coordinator at Choice Of New Rochelle In, you will play a critical role in connecting individuals experiencing homelessness with essential services and support. This position requires a compassionate and organized individual who can effectively communicate with clients, community partners, and internal stakeholders.

Key Responsibilities:

  • Establish and maintain relationships with clients, providing individualized support and guidance to achieve their goals.
  • Conduct thorough assessments of client needs, identifying areas for intervention and developing tailored care plans.
  • Collaborate with community organizations and service providers to ensure seamless referrals and support.
  • Monitor client progress, adjusting care plans as needed to ensure optimal outcomes.
  • Document all interactions and activities in a timely and accurate manner, utilizing case management systems to track client data.
  • Participate in regular meetings with the Homeless Outreach Supervisor to review caseloads, discuss challenges, and share best practices.
  • Develop and maintain productive relationships with key stakeholders, including mission-critical personnel at outside organizations.
  • Attend meetings and training sessions as necessary to stay up-to-date on best practices and industry developments.

Requirements:

  • High school diploma or equivalent required; degree in social work, psychology, or related field preferred.
  • Minimum 2 years of experience working with vulnerable populations, preferably in a case management or outreach capacity.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders.
  • Strong organizational and time management skills, with the ability to prioritize tasks and manage multiple caseloads.
  • Proficiency in case management software and other relevant tools.
