About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Amberwell Health Hiawatha. As a LPN, you will play a vital role in providing high-quality patient care and collaborating with our healthcare team to deliver exceptional outcomes.

Key Responsibilities
  • Assist patients with clinical services, including patient intake, documenting medical history, and preparing and administering injections and vaccinations.
  • Perform CLIA waived lab testing and other medical services within the scope of practice of the LPN.
  • Obtain, verify, and record health history of patients, gather pertinent health data through physical assessments and biological samples, and collect specimens for laboratory analysis.
  • Provide patient care as directed by physician or advanced practice clinician, making skilled judgments based on patient presentation and protocols in place.
  • Document assessments, interventions, patient/family interactions/responses, medications dispensed/prescribed, and test results in medical records.
  • Assist other healthcare professionals in performing exams, diagnostic procedures, treatments, and minor surgical procedures in the clinic setting.
  • Ensure accurate and timely documentation of patient information for quality reporting and data extraction purposes.
  • Coordinate and manage care of patients seen by provider, including tracking testing and referrals.
  • Stay informed of insurance requirements and guidelines for patient care, perform prior authorizations, and act as patient advocate.
  • Regularly monitor patients to promote wellness and health maintenance, including preventative and chronic care.
  • Participate in the development of the plan of care with other team members and safely implement that plan.
  • Promote wellness by providing patient education materials and communicating provider advice/instructions.
  • Schedule, coordinate, and communicate referrals to specialists and services ordered by the provider.
