Job Summary:

As a skilled Social Services Coordinator at Watertown Rehabilitation and Nursing Center, you will play a vital role in ensuring the social and emotional needs of our residents are met. Your expertise in developing and maintaining individualized care plans will make a significant difference in the lives of our residents.

Key Responsibilities:
  • Maintain and improve residents' ability to control everyday physical needs and mental/psychosocial needs.
  • Identify individual social and emotional needs and develop strategies to address them.
  • Participate in interdisciplinary care plan meetings to ensure the social and emotional needs of residents are met.
  • Work with the finance team to ensure residents are educated and proper documents are in place during their stay.
  • Assist with discharge planning and ensure necessary DME or home care is in place for a safe return to the community.
  • Demonstrate knowledge of age-specific developmental factors and plan social services programs accordingly.
  • Evaluate and assess the needs of prospective residents to determine appropriateness of facility placement.

Qualifications:
  • RN, BSW or MSW preferred, or Bachelor's degree in a related field.
  • At least one year of experience in the Social Work field required.
  • Experience in long-term care and/or sub-acute settings preferred.
