Job Description

At Watertown Rehabilitation and Nursing Center, we are seeking a skilled Social Services Director to join our team. In this role, you will play a vital part in providing exceptional care to our residents by developing and implementing individualized care plans that address their social, emotional, and physical needs.

Key Responsibilities:

  • Maintain and improve residents' ability to control everyday physical needs and mental/psychosocial needs.
  • Identify individual social and emotional needs and develop effective strategies to address them.
  • Participate in interdisciplinary care plan meetings to ensure comprehensive treatment of social and emotional needs.
  • Collaborate with the finance team to ensure residents receive necessary education and documentation during their stay.
  • Work with residents to ensure a smooth discharge process, including arranging necessary DME or home care.
  • Demonstrate knowledge of age-specific developmental factors and plan social services programs accordingly.
Requirements:

  • RN, BSW, or MSW preferred, or a Bachelor's degree in a related field.
  • At least one year of experience in the Social Work field, with a preference for long-term care and/or sub-acute settings.
Salary and Benefits

The estimated salary for this position is $65,000 - $85,000 per year, depending on experience. Our comprehensive benefits package includes health insurance, retirement plan, and paid time off.
