Job Summary

We are seeking a highly skilled and detail-oriented Patient Discharge Coordinator to join our team at KPH Healthcare Services, Inc.

As a Patient Discharge Coordinator, you will play a critical role in ensuring a smooth transition of care for our patients. You will work closely with our Kinney Home Services team to identify patients who are eligible for our discharge programs and coordinate the delivery of necessary services.

Key Responsibilities
  • Attend discharge meetings to confirm patient eligibility for our programs
  • Develop and maintain relationships with patients, families, and caregivers to ensure their needs are met
  • Coordinate daily with medical units to develop updated lists of patients' admissions
  • Populate patient information on our Kinney Home Services site
  • Meet with newly admitted patients to evaluate their needs and develop a plan for discharge
  • Assist with patient precertification and advocate on their behalf with commercial insurance or government health plans
  • Address queries and concerns from patient families and caregivers
  • Ensure timely delivery of discharge needs and coordinate with Kinney Home Services agencies
  • Follow instructions and complete tasks with a sense of urgency
  • Assess workload and establish Kinney Home Services products pick-up and delivery times
Requirements
  • High School Diploma or equivalent required; AS degree or higher preferred
  • 3 years of hospital or healthcare experience preferred
  • Ability to maintain confidentiality and handle sensitive patient information
  • Strong communication and interpersonal skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
Compensation

$17.00-$19.00 per hour

The final offer will be determined after careful consideration of multiple factors, including relevant skills, years of experience, and education.
