Primary Care Case Manager RN

We are seeking a dedicated Primary Care Case Manager Registered Nurse to join our team at Hopscotch Primary Care. Located in Shelby, NC, this role is an excellent opportunity for a skilled nurse to make a meaningful impact in the lives of our patients.
Job DescriptionThis dynamic role will involve managing end-to-end case management services for a defined panel of risk-stratified patients. As a Primary Care Case Manager RN, you will establish and maintain strong relationships across the care team, patient, family, caregiver, and broader provider and community ecosystem.
You will build relationships across the provider community and leverage data and team insights to refer care to preferred providers. Additionally, you will manage and plan for transitions of care, discharge, and post-discharge follow-up for patients transitioning to/from a hospital, ED, or SNF.
The ideal candidate will have a strong background in nursing, with a minimum of 2 years of clinical work experience and a valid, active Registered Nurse (RN) license in the State of employment. A Bachelor of Science Degree in Nursing (BSN) or an RN with a bachelor's degree in a related clinical field is also preferred.
Responsibilities- Care Coordination and Planning: Collaborate with clinical staff in the development and execution of the plan of care and achievement of goals. Report variations to the care team and take actions as appropriate.
- Patient Support and Advocacy: Actively advocate for the patient and serve as a resource for the family to maximize their ability to make informed, timely decisions and to enable patient outcomes.
- Social Services/Case Management: Coordinate the integration of social services/case management functions in the pre-acute, ER, acute, and post-acute setting.
- A minimum of 2 years of clinical work experience as a Registered Nurse.
- A valid, active Registered Nurse (RN) license in the State of employment.
- A Bachelor of Science Degree in Nursing (BSN) or an RN with a bachelor's degree in a related clinical field.
- Certification in Case Management is preferred.
