Care Coordination Nurse Specialist

As a Care Coordination Nurse Specialist, you'll play a critical role in our multidisciplinary team, working with patients, families, and healthcare providers to ensure seamless care transitions and optimal health outcomes.

Key Responsibilities:
  • Provide in-home and telephonic visits to patients at high-risk for hospital admission and readmission, focusing on prevention and early intervention.
  • Develop and implement comprehensive care plans, incorporating disease-oriented assessments, medication monitoring, health education, and self-care instructions.
  • Coordinate with multidisciplinary teams, including licensed practical nurses, social workers, and community health coordinators, to ensure patient-centered care.
  • Assess patient needs, identify community resources, and connect patients with support services to promote healthy aging and independence.
  • Collaborate with primary care physicians, specialists, and other healthcare providers to ensure timely and effective care transitions.
Requirements:
  • Associate degree in nursing required; bachelor's degree in nursing or related clinical field preferred.
  • Minimum 2 years' clinical work experience; 1 year of case management experience in community care highly desired.
  • Certification through Commission for Case Manager Certification (CCMC) or American Association of Managed Care Nurses (CMCN) a plus.
  • Strong interpersonal and communication skills, with ability to work effectively with diverse constituencies.
  • Proficient in Microsoft Office Suite products, including Excel, Word, PowerPoint, and Outlook.
