Job Summary:

UnitedHealth Group Inc is seeking a skilled RN Field Clinical Care Coordinator/HCBS to join our team. As a RN Field Clinical Care Coordinator/HCBS, you will play a critical role in assessing, planning, and implementing care strategies for patients in the community.

Key Responsibilities:

  • Assessment and Planning: Conduct thorough assessments of patients' needs and develop individualized care plans to ensure the most appropriate level of care.
  • Referral and Coordination: Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.
  • Care Management: Manage the care plan throughout the continuum of care as a single point of contact, communicating with all stakeholders to ensure quality coordinated care and services are provided expeditiously.
  • Patient Advocacy: Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.

Requirements:

  • Education and Licensure: Current RN unrestricted license in the state of TX.
  • Experience: 2+ years of experience working within the community health setting or in a healthcare role.
  • Skills: Familiarity with Microsoft Office, including Word, Excel, and Outlook, and reliable transportation for travel to visit patients in their homes and other settings.
  • Preferred Qualifications: 1+ years of experience with long-term care services and support, Medicaid, or Medicare, and knowledge of the principles of most integrated settings, including federal and state requirements.
