PHS operates WholeYouNYC (WYNYC), a coordinated community resource network that establishes reliable pathways between healthcare providers, health plans, and community organizations. WYNYC connects over 100 organizations offering diverse programs—such as food, housing, employment, health insurance, and sexual health services—across all five boroughs. These initiatives enable New Yorkers to achieve optimal health and reduce health disparities, ultimately advancing health equity.

PHS has been designated as the regional Social Care Network (SCN) for Brooklyn, Manhattan, and Queens, responsible for coordinating social care delivery across these areas.

Program Overview:

Family Connect is a dedicated sub-network within the WholeYouNYC framework, focusing on pregnant and newly parenting individuals in New York City. The program receives referrals from health and social service providers, assesses their needs, and connects them to home visiting programs, doulas, and various other supportive services through an electronic referral management system, Unite Us. The primary objective of Family Connect is to link families with essential services that address health-related social needs, thereby improving maternal and infant health outcomes.

Position Summary:

The Family Connect Program Supervisor will oversee a team of up to five Pregnancy and Parenting Navigators and/or Americorps members. This role involves managing the intake and referral process, ensuring timely and appropriate distribution of referrals based on urgency and need, and providing guidance on the use of referral systems. The supervisor will also coordinate on-site activities and community outreach efforts.

Key Responsibilities:

  • Provide regular supervision and oversight for a team of Pregnancy and Parenting Navigators and/or Americorps members.
  • Distribute referrals to staff based on urgency and language proficiency.
  • Support staff in addressing challenges and identifying new needs for the families they serve.
  • Ensure accurate use of referral platforms and effective communication with referral sources.
  • Evaluate staff performance and offer coaching to enhance service delivery.
  • Participate in team meetings and contribute to collaborative discussions.
  • Build and maintain relationships with community partners.
  • Coordinate staffing for local outreach initiatives.
  • Assist with the organization of community support activities.
  • Participate in the recruitment, evaluation, and training of navigators.

Qualifications:

  • Bachelor's degree (or equivalent) with 1-2 years of experience in public health or resource navigation.
  • Demonstrated commitment to maternal and child health and community support.
  • Bilingual candidates preferred, with proficiency in English and another language commonly spoken in NYC.
  • Strong organizational skills and the ability to manage multiple tasks effectively.
  • Excellent communication skills, with a focus on empathetic support for team members.
