Position Overview:

An experienced Registered Nurse (RN) tasked with the comprehensive management of patient care, including the delivery of direct services while adhering to a designated productivity standard. This role involves planning, organizing, and directing all patient care activities for individuals within the assigned caseload, in alignment with current standards and regulations, ensuring the highest quality of care is consistently upheld.

Key Responsibilities:

  • Coordinate the interdisciplinary team to execute the established patient care plan.
  • Ensure effective management of visit utilization and control of associated expenses.
  • Participate in agency-wide orientation and competency evaluations, providing consultation to clinical staff based on specific expertise.

Schedule: Full-Time Day Shifts: Monday-Friday with Rotating Weekends

Education Requirements:

  • Graduate of an accredited School of Nursing

Licensure & Certifications:

  • RN - Registered Nurse of California
  • BLS - Basic Life Support Healthcare Provider
  • Valid Driver's License Class C
  • Automobile Insurance

Experience:

  • Minimum of 2 years of recent relevant experience

Skills and Competencies:

  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Sound nursing judgment in planning and delivering patient care.
  • Knowledge of pathophysiology and accepted treatment protocols for common home care diagnoses.
  • Understanding of safety and infection control principles.
  • Strong clinical assessment capabilities.
  • Effective communication skills with patients and their families.
  • Ability to maintain constructive relationships with internal and external stakeholders.
  • Proficient in written and verbal communication.
  • Understanding of palliative care for terminally ill patients.
  • Capability to identify and respond to unsafe or emergency situations appropriately.
  • Meet company productivity standards as outlined by management.
  • Problem-solving skills to address unique challenges as they arise.
  • Ability to navigate a demanding work environment with competing priorities.
  • Maintain a professional image and engage in self-directed learning for professional development.
  • Responsible for maintaining all required licensure and certifications.
  • Reliable transportation is essential.
